How to Apply

Application form available with prospectus. It can be obtained from college web-site / the office of admission cell, Dr. PDNI Amravati. Read the prospectus carefully for proper information before filling the application form. Thefilled-inform should be submitted to the office of admission cell, Dr. PDNI Amravati.The last date of submission of form will be displayed on the web-site / college notice board.

Documents Required

Thefilled-inapplication form should be submitted within stipulated time period. However, a filled-in form shall be considered only when it fulfils the following criteria –

1. Correct and completely filled in application form
2. Received by the institute on or before the last date of admission
3. Accompanied by attested copies of relevant documents

• School / college leaving certificate (duplicate TC not admissible)
• Secondary school certificate and secondary school mark sheet
• Higher Secondary school certificate, and higher secondary school mark sheet
• Medical fitness certificate
• Nationality certificate / domicile certificate / age certificate
• Caste certificate, caste validity and non-creamy layer certificate for the applicants belonged to reserved categories. Whereas no certificate of non-creamy layer is required for the applicants belonged to SC/ST categories
• Migration certificate in case of applicant passed last exam from a board other than Maharashtra state / university other than MUHS Nashik
• Nursing council registration certificate with latest renewal, diploma / degree certificate, and the certificates showing statement of marks for the applicants seeking admission to PB B. Sc. (N) or M. Sc. (N) course
• Gap certificate, if the applicant had break in continuity of education
• CET Cell registration form and CET score card for B. Sc. (N) nursing admission
